Media and Promotional Consent 

I consent to the capturing and use of photographs, audio and video recordings taken of me or a child (under 18 years) I am the parent or carer of. 

By signing this form, I give permission for LiveWest to use the photographs or recordings in or on:

• The internet including social media platforms including third party websites such as, other housing associations, shares on social media, a professional photographer, or our partners we work with. 
• Printed and digital publications and promotional materials including reports, publications, display material, websites and other forms of literature and electronic media and posters.
• Other channels used to promote the work of LiveWest and with our partners.
• We may use audio recordings and quotes to support the creation of and promotional case studies.
• Press releases to external sites such as media outlets, regional and local media, and their social media accounts. This will include third party websites such as other housing associations, shares on social media, a professional photographer, or any LiveWest partners.

I acknowledge that I may withdraw my consent at any time by contacting LiveWest to let them know not to use the photos or recordings anymore. If I do withdraw consent, I understand that the photos or recordings may already be in the public domain and therefore could potentially be reproduced, altered, or re-used by anyone outside of LiveWest.

I acknowledge that the copyright of the photograph/video/audio/quote belongs to LiveWest.

Under the 2018 Data Protection Act, your rights include:

• Your consent (to the publication of you or your child’s/children’s images) can be withdrawn at any time.

• Your images/recordings will not be used for any other purpose without your further consent.

• The contact information you have provided will only be used to contact you about these images/digital recordings.

Conditions of use:

• We will not include personal e-mail, postal addresses or telephone numbers on video, our website or in printed publications and advertising.
• We may use group or class images with very general labels such as ‘Residents at LiveWest homes.’
• We may use the image in literature unrelated to when the photograph was taken.
• We may include your full name with the image.
• The use of this ‘story’ may be used in the future but in an anonymised way without obtaining further consent.
• We will retain your consent form for six years; If I do withdraw consent, I understand that the photos or recordings may already be in the public domain and therefore could potentially be reproduced, altered, or re-used by anyone outside of LiveWest.
• These images and recordings may also be used by LiveWest and with our partner organisations such as other housing associations, partner shares on social media, a professional photographer, or the partners we work with.
